【招生公告】114學年度轉系、所申請公告Application for Changing Major in Academic Year 2025
* 擬申請轉入光電所同學，申請流程如下*
（一）申請表請至「校務資訊系統」填寫後將紙本印出。
（二）請先送指導教授簽章，再送光電所辦陳核主管。
（三）需繳交資料：請依學校註冊組公告之轉系所時間(截止日6/11前)繳交資料至以下單位，逾期不受理。
- 需繳至 註冊組 的資料：
轉系所申請表（必須繳交，請登入校務資訊系統填寫後交送達註冊組辦公室)
- 需繳至 光電所 的資料：(台達館517室)
- 原就讀所所主管同意之證明文件。
- 前一學年之成績單及名次證明。
- 原碩、博士班入學考試成績單。
- 碩士班研究生應繳大學部畢業成績單及名次證明；博士班研究生應繳大學部及碩士班畢業成績單及名次證明。
- 讀書計畫-含【轉所(組)之緣由】。
（四）提出申請之前，建議請先與本所研究相關領域之教授進行面談，以確認轉所之可能性。

二、 申請流程Process：
（一） 申請表請至「校務資訊系統」填寫後將紙本印出https://www.ccxp.nthu.edu.tw/ccxp/INQUIRE/
Please go to the Academic Information Systems to fill in the application form and print it out.
（二） 學士班：送請原就讀學系、院學士班主任及導師簽註意見。
For undergraduate students: Bring the application form to your original department. The application form should be duly signed by the Chair and the mentor.
（三） 研究生：送請原就讀系、所、學位學程指導教授及主管簽章。
For graduate students: Bring the application form to your original department/institute. The application form should be duly signed by the Chair and the advising professor.
（四） 學生申請轉系須繳交各學系、所、院學士班之審查資料說明，請點選參照網址。若未列出，建議直接向系所詢問。
About the required application packets to the intended department, please refer to the website.
（五） 請於6月11日(三)前將轉系所申請表送達註冊組辦公室，審查資料請依各系所規定時間送達欲申請的各學系、所、院學士班辦公室，逾期不受理。
The application forms shall be sent to the Division of Registration before June 11, 2025. The required application packets shall be sent to the intended department’s/institute’s office before deadline. Late applications will not be accepted.
